What Is This Program?

CMS seeks applications for State Planning Grants for promoting continuity of care for individuals who are inmates of a public institution and are eligible for medical assistance under the state Medicaid program or the Children’s Health Insurance Program (CHIP). Funds may be used for addressing operational barriers and improving systems for continuity of care following incarceration. These planning grants also work towards operationalizing new requirements which prohibit the termination of eligibility for Medicaid and CHIP due to incarceration for all Medicaid and CHIP enrollees beginning January 1, 2026.
